It is mandatory for each passenger to truthfully complete the information provided to the Directorate of Identification, Immigration and Foreigners, the Custom General of the Republic and the Ministry of Public Health.
Once the form is filled up, you will receive a PDF with the QR Code via email, which you should show either on a device or on a printed copy to the authorized personnel in Cuba.
